  • SSA-3441 (Disability Report – Appeal)
  • SSA-827 (Authorization to Disclose Information)


These can be submitted online at ssa.gov/benefits/disability/appeal.html, by mail, or in person at your local Wyoming SSA office.


4. Provide Additional Evidence

If your condition has worsened or you have new medical records, submit them with your appeal. New evidence can significantly strengthen your case.
